Custom Settings
There are a number of options for establishing the disclosure form as shown on the left side of the screen. Here's a description of each option: 

Use First Inventor - Used to set a first inventor, as shown on the patent application. The inventor creating the disclosure is automatically set as the first inventor. However, the inventor can designate a first inventor, provided the checkbox is checked, when adding inventors to the disclosure.

Can Add Inventor - If checked, the inventor creating the disclosure form can add inventors.

Use Lead Inventor - Used to set a lead inventor. The inventor creating the disclosure is automatically set as the lead inventor. However, the inventor can designate a lead inventor, provided the check box is checked, when adding inventors to the disclosure.

Inventor Contribution - If checked, the inventor creating the disclosure is required to enter in a contribution amount and date when adding additional inventors.

Committee Custom Fields - If any custom matter fields (https://support.appcoll.com/customer/portal/articles/1841875) have been added to your account, they can be displayed within a disclosure form. Please note, these fields are only visible to account admins.

Admin Custom Fields - This is the same as Committee Custom Fields, however visibility is limited to account admins, independent if they are committee members.

Admin Can Always Edit - By default, a disclosure form cannot be edited by anyone other than the inventors and then only why the disclosure is in an editable status. For example, inventors cannot edit a disclosure once it has been sent to the committee. In normal flows, inventor can only edit a disclosure before being submitted or once sent back to them for a rework. Checking this check box allows account admins to edit a disclosure at any point in the flow.

Generate PDF - If checked, a PDF of the disclosure is generated when the disclosure is saved and subsequently edited. The PDF is attached to the disclosure and accessible by parties having access to the disclosure. The disclosure form has version control. Thus, the latest version is visible.

Documents Required - If checked, at least one document must be submitted with the disclosure such as lab notes, white papers, code, flow charts, presentations, etc.

Disclosure Form

General Instructions - At the top of the form is the general questions text field. Enter instructions for the inventors with respect to completing the disclosure form.

Disclosure Questions - There are 48 possible questions that can be added to the form. Each question can be in the form of a date, text field, drop down menu or a combination of all these. Enter a name and description for each question. Provide sufficient detail for the inventor to fully understand the question being asked. Any question can be in the form of a drop down menu, by adding each desired value as a separate line in the "Restricted Values" section of the question.

Please Note: We highly recommend leaving a couple of blank lines between each question. Presently, questions cannot be easily re-ordered or new ones inserted between two consecutively populated fields. Thus, leaving blanks between questions allow for adding new questions down the road without rearranging existing questions.

Required Fields - Disclosure questions can be set as required by clicking the check box next to each disclosure field.
